The Queensland Government requires all Uber driver-partners to hold a current “Booked Hire Driver Authorisation (DA)” with the Department of Transport and Main Roads. As part of your DA application you are also required to get a commercial standards medical check or also known as an Uber Driver medical assessment. This also includes senior drivers.

How It Works?

To complete your drivers medical assessment, follow these simple steps:

  • Step 1: Book an appointment with one of our friendly doctors.
  • Step 2: Print blank copies of both the F3712 and F3195 forms.
  • Step 3: Complete part 1 of both documents prior to your booking.
  • Step 4: Bring both forms with you to your appointment.
  • Step 5: Your doctor will then complete your health assessment.

Once you have a signed copy of both the F3712 and F3195 from your medical assessment, you will then need to complete a Driver Authorisation Form. Please also print a copy of the relevant form:

  • First Time Uber Applicants: Download & Complete Form - F2978
  • Renewing Uber Applicants: Download & Complete Form - F2979

You can then submit your application to your closest Department of Transport and Main Roads. It typically takes 2-3 weeks for this to be approved and it’s a good idea to keep a copy of these forms for yourself as Uber will also require a copy. Once approved, upload your drivers authorisation to your Uber account. This should have you on your way to becoming an Uber driver and generating income.

Our Fees

For both new and existing patients of the practice, Uber driver medical assessments cost $198 (inc GST) excluding treatment room fees. All services for Uber driver medicals are privately billed. As the assessment is work related, please be aware that there is no Medicare rebate for this consult. We recommend you retain a copy of receipt and if the cost incurred is classed as a business expense with your tax agent.

Please Note: If you do not have access to a printer or have issues with downloading the medical form, please arrive 15-20 minutes early to your appointment and our reception team will be able to provide you with a copy for you to complete prior to your booking.

During your Uber medical check, our healthcare professionals will evaluate your overall health and fitness to drive. The assessment typically includes a review of your medical history, a physical examination and simple tests such as testing your vision as well as hearing checks. The goal is to ensure that you meet the health and safety standards required for driving passengers, however, specific requirements may vary depending on your driving situation. 

On average, your driving medical for Uber will take between 15-20 minutes to complete. While we strive to ensure that your appointment runs on time, if you have not completed part one of your certificate or are unable to arrive on time, the length of time it takes to complete your medical assessment may increase.

 

Unfortunately, Medicare does not cover the cost of medical assessments required by private employers, including Uber. These assessments are considered occupational health services, which fall outside of Medicare's coverage. However, some private health insurance plans might offer partial reimbursement for such expenses and so it's worthwhile consulting with your health fund.

Depending on whether you are a new Uber driver or renewing your drivers authorisation will depend on whether you need the F2978 or F2979 form. You will not need both, just one or the other depending on your situation.
