Comprehensive health assessments are offered at Narangba Station Medical Centre to promote the overall well-being of patients. The skilled general practitioners at our centre provide thorough evaluations to assess various aspects of health, including physical, mental, heart health and preventive care.

40-49 year old health assessments
Understand the risks, and live better
A visit to our practice and a consultation with our team is all that’s required. It will take 45-60 minutes but could make a world of difference to your future health.
We will:
- update your medical history
- ask questions to assess your current health and identify risk factors for developing diabetes
- complete a brief physical examination, where we measure your weight, waist circumference, blood pressure and blood sugar (additional tests may be required if requested by your doctor)
- take a blood test (depending on the outcome of your assessment)
- provide you with practical advice and information on how to make lifestyle changes that will improve your health and reduce your risk of developing diabetes or chronic disease in the future.
Additionally, depending on the results of your assessment, you may also be eligible for further government funded plans and programs including:
- GPMP and TCA (Care Plan and Team Care Arrangements)
- SPMC Diabetes Care Program
- 45-49 Year Health Check
- Mental Health Care Plan
45+ year old health assessments
Understand the risks, and live better
All adults aged 45 and over and Aboriginal and/or Torres Strait Islanders aged 30 and over are eligible for a Healthy Heart Check. A Healthy Heart Check is a risk assessment and screening tool used to calculate your risk of cardiovascular disease (CVD). These appointments are also bulk billed at Narangba Station Medical Centre.
Aboriginal and Torress Strait Islander peoples health
If you identify as an Aboriginal or Torres Strait Islander person and have a Medicare card, you can get a free health check every year. This health check is important. It can help identify whether you’re at risk of illnesses or chronic conditions. It’s much easier to prevent these than to treat them, so catching them early can make a big difference.
If you have a chronic condition, your health check can help make sure you are getting the medication and care you need. You can also ask your doctor about anything you’re worried about or need help with.
Your doctor can only do this health check if you agree. You can have it every 9 to 12 months, and it’s free. After your health check, your doctor can refer you to up to 10 free follow-up services if needed, like:
- a specialist or another health service
- a counsellor
- a service that works to prevent chronic conditions.
GP Chronic Disease Management Plan
Doctors at Narangba Station Medical Centre work with patients to achieve optimal outcomes through a GP Chronic Disease Management Plan. In general practice, a care plan refers to the process for setting and achieving goals. This Australian care plan aims to support individuals with chronic medical conditions, and a GP works collaboratively with the patient to develop a comprehensive management strategy tailored to their specific needs. The plan typically involves
- Assessment of the patients condition,
- Outline of the GP’s goals for the patient,
- Addressing and consideration for the patient’s needs and goals
- Planning the treatments and actions to achieve the goals including medical and allied health services,
- Structured reviewed and monitoring
Team Care Arrangement Plan
A Team Care Arrangement Plan may be implemented by your doctor if numerous healthcare professionals are involved in managing your condition, and your condition requires multidisciplinary care. The goal of this approach is to create a coordinated and integrated approach to the patient's care, ensuring that all healthcare providers involved are aware of the patient's needs, goals, and treatment plans.
