Need a Vaccine Preventable Diseases Certificate?

Our GPs are here to help. If your role with a high-risk industry requires documented evidence of immunity or vaccination, we can guide you through the entire process. From immunity assessments and blood tests to providing required vaccinations and completing your VPD Evidence Form, we make it simple and stress-free.

What are vaccine preventable diseases?

Vaccine preventable diseases (VPDs) are infectious illnesses that can be significantly reduced or avoided through a duty of care and vaccination. In both healthcare and other high-risk industries, immunisation plays a vital role in protecting not only your health, but also the wellbeing of patients, colleagues, and the wider community.

The 6 specified vaccine preventable diseases under the Queensland Health guidelines include:

Depending on your nature of work, other employment-required vaccine preventable diseases may also include:

Depending on your medical history and existing immunity, you may not need all six vaccines. In some cases, documented evidence of previous vaccination or blood test results showing immunity can meet the criteria.

How it works?

Our VPD assessment includes reviewing your medical and immunisation history, conducting relevant blood tests (if required), and administering any necessary vaccinations. Once eligible with the necessary test results and vaccinations, we will assist eligible patients  to complete the Vaccine Preventable Diseases Evidence Form for their employer.

To get started, please follow these simple steps below:

  • Step 1: Book an initial consultation with our doctors.

  • Step 2: Print a copy of the VPD evidence form (Download).

  • Step 3: Attend your appointment and bring the form with you.

  • Step 4: On the day of your appointment, your GP will conduct a comprehensive medical assessment tailored to your requirements.

  • Step 5: We will provide you with a vaccine schedule with follow up appointments as necessary to complete the requirements.
  •

If you’re unable to print the form ahead of time, please arrive 15–20 minutes early, and our reception team can provide a copy for you to complete before your appointment.

Frequently Asked Questions

Questions about vaccine preventable diseases? No problem! Here are the most commonly asked questions we receive

Vaccine preventable diseases are illnesses that can be significantly reduced or avoided through immunisation. Under Queensland Health guidelines, the six specified VPDs include measles, mumps, rubella, varicella (chickenpox), pertussis (whooping cough), and hepatitis B.

Anyone applying for a role within Queensland Health, or other industries where there’s a higher risk of transmitting infectious diseases, may be required to complete a VPD assessment and provide documented evidence of immunity or vaccination. You will be notified by your employer of the requirements prior to starting your role.

In most cases, we can complete your VPD assessment and documentation promptly, especially if you already have records of previous vaccinations. If additional vaccines or blood tests are needed, the process may take a little longer. However, our team will guide you every step of the way to ensure everything is completed efficiently.

Please bring your printed VPD Evidence Form, any previous vaccination records, and a valid Medicare card (if you have one). If you don’t have the form printed, arrive 15 to 20 minutes early and our reception team can provide a copy.

In some cases, yes. However, depending on the number and type of vaccines required, your GP may recommend a schedule over multiple visits for safety and effectiveness. We will provide a clear plan during your initial assessment.
