If you suffer from persistent pain or have a chronic condition, you may be eligible for a GP Chronic Condition Management Plan (GPCCMP). A GPCCMP is a comprehensive, structured and Medicare-funded plan that aims to help you access the treatment, support and referrals that you need.
Patients with a number of chronic conditions are eligible for this plan, including osteoporosis, osteoarthritis, chronic pain syndromes, fibromyalgia, chronic fatigue, diabetes, heart disease and other chronic conditions that have lasted or are expected to last for six months or more.

What Is a GP Chronic Condition Management Plan?
A Chronic Condition Management Plan (previously known as a GP Management Plan) details your health needs, goals and your ongoing care.
Under Medicare, subsidised access to a comprehensive range of health services is available to eligible patients. You can receive help for things that cause you chronic pain and see a number of practitioners without the need for significant out-of-pocket costs.
