Health Checks & Medical Assessment Services
At Narangba Station Medical Centre, we provide a range of health assessments to ensure your health is carefully monitored and managed. Whether you're coming in for a routine check-up or a more specialised assessment, our team is dedicated to delivering thorough, personalised care. Each assessment is tailored to your specific health needs, helping to identify risks, prevent illness, and manage ongoing conditions. Below is an overview of the key assessments we offer.
General Health Assessment
Our general health assessments provide thorough check-ups for patients of all ages. This may include evaluating overall health, discussing lifestyle factors such as diet and exercise, and assessing any symptoms or concerns you may have with your assessing doctor.
Over 75 Health Assessment
Tailored for those over 75, this assessment ensures you’re receiving the appropriate care for your stage of life. It focuses on mobility, mental health, medication management, and overall well-being to support healthy ageing.
Indigenous Health Assessment
Specifically designed for our Aboriginal and Torres Strait Islander patients, this assessment considers cultural factors and specific health risks to ensure holistic, culturally appropriate care.
Heart Health Check
This assessment focuses on your cardiovascular health, identifying risk factors like high blood pressure, cholesterol levels, and family history of heart disease. It’s especially important for individuals over 45 or those with pre-existing risk factors.
The Importance of Annual Health Checks
Annual health checks are essential to maintaining long-term well-being. These regular assessments give us a chance to catch potential health issues early, monitor ongoing conditions, and provide you with advice on how to stay healthy. Our focus is not just on managing illness but preventing it. By prioritising an annual health check, you’re taking an important step toward proactive health management.
What To Expect?
Most of our health assessments at Narangba Station Medical Centre follow a similar process, tailored to meet your specific health needs. While some assessments may include specialised tests or screenings, you can generally expect the following steps during your visit:
1. Initial Consultation
Your assessment generally begins with a discussion of your medical history, lifestyle, and any current health concerns. This is a crucial step for our GPs to understand your health background, including any medications you're taking or family history of medical conditions.
2. Physical Examination
A thorough physical examination may be conducted, covering vital signs such as blood pressure, heart rate, and respiratory health. This step helps to assess your overall physical condition and identify any immediate health concerns.
3. Screening Tests
Depending on the type of assessment, you may undergo specific screening tests. These could include blood tests, cholesterol checks, blood glucose monitoring, or heart-related tests like an ECG. The results provide valuable insights into your current health status.
4. Preventive Care Discussion
Our GPs may discuss preventive health measures based on your results. This might include lifestyle recommendations such as diet changes, exercise routines, or ways to reduce risk factors for certain conditions like heart disease or diabetes.
5. Management PlanÂ
If any health concerns are identified, your doctor will work with you to develop a management plan. This could involve further tests, such as 24-hour Blood Pressure Monitoring or Holter Monitoring, specialist referrals, or ongoing monitoring.
6. Follow-Up
In some cases, and where required, a follow-up appointment may be scheduled to reassess your progress. This allows your doctor to ensure that any treatment recommended or management plan put in place, is effective and working for you.
Team Care & Management Plans
We recognise that managing chronic conditions requires a comprehensive, ongoing approach. Our Care Plans are designed to support patients with long-term health conditions by providing a structured plan to manage and coordinate their care. These plans ensure that you receive the best possible support and guidance from our GPs and allied health professionals. Below is an overview of our key Care Plans we offer:
GP Chronic Disease Management Plan
For patients with chronic medical conditions such as diabetes, asthma, heart disease, or arthritis, the GP Chronic Disease Management Plan (CDM) offers a tailored approach to managing your health. This plan outlines your treatment goals and the steps you need to take to manage your condition effectively. Your GP will work with you to monitor your progress, adjust treatments as needed, and provide ongoing support to improve your quality of life.
Team Care Arrangement Plan
Many chronic conditions require input from various healthcare providers. Your doctor may put in place a Team Care Arrangement (TCA) Plan which coordinates your care between your GP and allied health professionals, such as physiotherapists, dietitians, and psychologists. The goal of this approach is to create a coordinated and integrated approach to the patient's care, ensuring that all healthcare providers involved are aware of the your needs, goals, and treatment plans.
